This time, command modules are de-lhs'ed.
The small change in Preproc.hs is just to make the code clearer.
In the patch "get rid [...] for commands" I also remove a duplicate
section of the "show" commands, and add "show index" that was not there.
Printer.lhs remains unchanged, but probably some parts of it are now
implemented in some string/text library, and the de-lhs-ization could
be done with a refactoring.
These patches change code all over the place, and the compilation,
tests and manual generation are going fine, so I'm screening them now.
